ADHD evaluation is a comprehensive clinical process. It gathers detailed behavioral, developmental and functional history relevant to Attention-Deficit/Hyperactivity Disorder and related conditions.
After you book, you'll receive the Comprehensive ADHD Diagnostic Intake Questionnaire securely through the patient portal. It typically takes 25-40 minutes; please answer honestly, with real examples from your daily life.
Your responses are reviewed alongside a clinical interview and any relevant records to support diagnostic clarification and treatment planning.
What the evaluation considers
- Developmental, learning and neurological history
- Cardiac history and vital signs relevant to treatment safety
- Sleep quality and fatigue
- Substance and caffeine use
- Mood, anxiety and trauma that can affect focus
